Informed Consent Agreement And Client Contact Information Form For Quantum Energetic Transformation & Healing Sessions

Please read and agree to the following before participating in your session:

I request that Cathy Hazel Adams conduct sessions with me for the purpose of changing my mental and emotional energetic patterns in order to release energetic blockages and help me shift into my full potentials for an authentic, joyful, healthy, and fulfilling life.

I understand and agree that:

-The sessions do not include physical manipulation of any kind.

-These sessions are not intended as diagnosis, prescription, treatment, therapy or cure 

  for any disease or disorder, mental or physical, and are not a substitute for

  regular, prudent medical care.

-Every session and every client is different, and my changes will appear in the way

  that is unique and specific to me, when and in the way I am ready, willing and able to

  accept and experience them. Cathy Hazel Adams does not control when or in

  what way my changes appear and cannot guarantee specific results.

-My changes may be subtle and/or astounding, may appear in unexpected ways, and

 may be apparent immediately and/or manifest over days, weeks and months.

-My transformational sessions may reveal other unresolved issues, and that

 as I shift, I  may temporararily experience uncomfortable emotions, memories, physical 

 responses or situations.

-More than one session may be needed to accomplish my best possible results:

 If my mental, emotional and/or physical difficulties are severe, chronic or long-standing,

 I understand that I am strongly urged to commit to 2 or 3 sessions to allow myself

 maximum opportunity to shift long-held and deeply entrenched patterns of thought,

 emotion, expectation, experience, and bodily memory into a new way of being.

-Cathy Hazel Adams is a practitioner of intuitive, quantum multidimensional transformation and energy healing, a conscious co-creation specialist, and a Matrix Energetics® Certified Practitioner, and that she is not licensed by any state as a Medical or Mental Health Care Practitioner.  Cathy Hazel Adams is not a legal or financial professional and does not profess to give advice in these areas.

-The methods, procedures, tools and modalities used in these sessions are based in

  universal creative law and are not part of any recognized religion.

-Sessions cancelled less than 24 hours in advance may incur a late cancellation fee.

-I understand that I am responsible for my own health, and for how I interpret, apply and

 experience the information and shifts that come up during our sessions, and for my

 own outcomes.  I hold harmless Cathy Hazel Adams, her heirs and assigns, and Cathy

 Hazel Adams, LLC, from any and all liability, including financial.

-I certify that I am 18 years of age or older.

Specific results cannot be guaranteed, and I do not claim to treat, diagnose, prescribe, heal or cure, as by law only licensed medical practitioners are authorized to claim to do these things.  However, if you experience changes that feel miraculous to you, you are free to enjoy them and call them whatever you please.
